It's even simpler than was stated here.  "Chicago center, Lancair _______ has begun an emergency descent to 10,000'"
But, the first step was to don the oxygen mask. (A "quick doning mask" even better.)
During the descent, ATC can assist you with a vector for terrain, but then there is time for such niceties as you are crow baring to earth.
This is a true emergency and no pleases or requests or thank you's are in order and only get in the way.
ATC works for us.  We are PIC.
